Concrete Imaging Services (CIS) Client Terms

Ground Penetrating Radar is used to locate targets such as post tension cable, rebar, conduit, voids, PVC pipe, and check depth. GPR uses ultrasound type waves by directly scanning the concrete surface and interpreting the results on the designated screen. After area(s) are scanned the targets are located and marked on site. If additional reports are needed other than an onsite analysis and onsite target marks you must notify the office upon ordering the job, or prior to dispatching the CIS crew. The sole purpose of GPR is to locate targets. Though the employees and equipment of CIS will interpret the information to the best of their ability, the GPR is not to determine what the target is or any exact depths.

CLIENT RESPONSIBILITIES
• Specifically designate all imaging locations with at least 6 inches of clearance on all sides.
• Prevent all cell phone use within 30 feet while imaging is being conducted.
• Imaging surfaces and the surrounding area must be completely dry; damp does not mean dry.
• Concrete to be imaged must be completely cured, a minimum of 2 months.
• Imaging locations must be clear of all materials, including dust, dirt, drywall, mud, nails, and metal of any kind.
• Access to imaging locations must be available, clear of tools, materials and equipment.
• Client is advised to rely on all available information and to not rely solely on CIS imaging services.
• The undersigned represents that he/she has full and complete authority to execute this agreement.

OTHER TERMS AND CONDITIONS
•If site preparations are not to the operator’s satisfaction, CIS reserves the right to reschedule and charge a $300.00 mobilization charge, or to charge an additional fee of $200.00 per hour for standby time on top of the said mobilization charge.
•If imaging locations are located on walls or ceilings, or involve pan decking, imaging set-up and interpretation will take additional time and incur additional costs.
•CIS, makes no warranties, expressed or implied, regarding the nature or quality of the imaging results, including the interpretation of data, and hereby disclaims all responsibility for damages that may result from reliance on its services.
•CIS results are provided solely for illustration and informational purposes.
•CIS makes no recommendations regarding the Client’s use of or reliance on CIS services.
•Client agrees to pay all charges within Net 30 Days unless otherwise noted on Customer Information, and agrees to pay a 3.5 % per month finance change on all past due balances. Client agrees to pay all collection costs incurred by CIS, including reasonable attorney fees whether or not litigation has commenced. An additional charge of $250.00 shall apply to each job on which CIS files a Mechanic’s Lien.
•No work will be scheduled or performed until an executed copy of this agreement is received by CIS.
